Change of Program Request for Graduate Students
The Graduate Change of Program Request is required when graduate students wish to change their majors. Students should meet with a graduate academic advisor in order to get this completed; please see Eric, Karen or Emily for further information. More information about changing programs may be found at the Office of the Registrar.

Current PhD Student Applying to Masters Level
PhD students who have fulfilled the requirements for the MS degree may apply for dual status and receive a diploma for the MS degree while continuing with the PhD program. Please deliver the completed form to Eric Moden, ECSS 4.231.

Deferment of Admission
If a student is unable to attend the semester for which he/she has been admitted, the Deferment of Admissions form must be submitted to the Office of Enrollment Services to indicate the semester of future enrollment. More information about deferment of admission may be found at the Office of Admission and Enrollment Services.

Individual Instruction Request (Independent Study)
Students interested in independent study courses should contact Eric Moden, ECSS 4.231.

Registration Add/Drop Form
Students should use this form each semester to enroll, add, drop, or withdraw from classes. PhD, thesis, teaching and research assistants should submit this form to Eric Moden, ECSS 4.231. All other MS students should submit this form to the graduate CS staff in ECSS 4.232 or ECSS 4.230.

Repeated Course Adjustment Form
When repeating a course, students must submit a Repeated Course Adjustment form to the Office of the Registrar.

Request for Recommendation Letter Form
When applying to graduate school, three letters of recommendation must be provided from individuals who are able to judge your probable success in graduate school.

Transfer of Graduate Credit Request Form
After the completion of 9 graduate hours at UTD, graduate degree-seeking students may apply for a transfer of graduate course work during the transfer/waiver period held each semester. Additional information about the transfer of graduate course work may be found in the Newly Admitted Student section of the FAQ page.

Travel Request Form
Prior to any travel, teaching and research assistants must submit this completed form to the Department of Computer Science. Please deliver this form to Eric Moden, ECSS 4.231.

Waiver of Prerequisite Course Request Form
After enrolling at UTD, degree-seeking students may apply for waivers of assigned prerequisite courses during their first semester of enrollment. More information about waivers may be found in the Newly Admitted Student section of the FAQ page.
