Change of Program Request for Graduate Students
The Graduate Change of Program Request is required when graduate students wish to change their majors. Students must bring
this form to the ECS South 2.502 advising suite for their advisor's signature. More information about changing programs
may be found at the Office of the Registrar.
Current PhD Student Applying to Masters Level
PhD students who have fulfilled the requirements for the MS degree may apply for dual status and receive a diploma for
the MS degree while continuing with the PhD program. Please deliver the completed form to Judy Patterson or Emebet Sahle
within the ECS South 4.801 suite.
Deferment of Admission
If a student is unable to attend the semester for which he/she has been admitted, the Deferment of Admissions form must
be submitted to the Office of Enrollment Services to indicate the semester of future enrollment.

Graduation Checklist
Students should follow these instructions before applying for graduation. View the UTD Graduation Website to find
additional information about graduation.
Individual Instruction Request (Independent Study)
Students interested in independent study courses should submit this completed form to Judy Patterson or
Emebet Sahle within the ECS South 4.801 suite.
Registration Add/Drop Form
Students should use this form each semester to enroll, add, drop, or withdraw from classes. PhD, thesis, teaching and
research assistants should submit this form to Judy Patterson or Emebet Sahle within the ECS South 4.801 suite.
All other MS students should submit this form to the graduate CS staff in the ECS South 2.502 advising suite.
Repeated Course Adjustment Form
When repeating a course, students must submit a Repeated Course Adjustment form to the Office of the Registrar.
Request for Recommendation Letter Form
When applying to graduate school, three letters of recommendation
must be provided from individuals who are able to judge your probable success in graduate school.
Transfer of Graduate Credit Request Form
After the completion of 9 graduate hours at UTD, graduate degree-seeking students may apply for a transfer of
graduate course work during the transfer/waiver period held each semester. Additional information about the

Travel Request Form
Prior to any travel, teaching and research assistants must submit this completed form to the
Department of Computer Science. Please deliver this form to Judy Patterson or Emebet Sahle
within the ECS South 4.801 suite.
Waiver of Prerequisite Course Request Form
After enrolling at UTD, degree-seeking students may apply for waivers of assigned prerequisite courses during
their first semester of enrollment. More information about waivers may be found in the Newly Admitted Student section of the FAQ page.
